As a developing strand of the Outcomes UK portfolio of clients, we are working with a number of clients in the Adult Health and Social Care sector.

We offer a range of services including the following interim management, interim social work and project management offerings:

• Making the transformation agenda a reality; providing interim and project managers to implement the personalisation agenda throughout organisations, from policy and procedure development to frontline practice
• Developing commissioning strategies to meet the personalisation agenda
• Clinical Governance, including CQC registration / inspection support
• Culture change management, through use of Outcomes Based Accountability
• Developing & reengineering Care Pathways to meet efficiency requirements whilst remaining fit for purpose
• Service evaluations, reviews, redesign
• Linking with Children's Services to ensure cradle-to-grave, person centred planning and a sound transition process
• Adding capacity to Social Work Teams to work to help review care packages in alignment with the national target of 30% of eligible service users to be in receipt of personal budgets by April 2011

Partner agencies

Outcomes UK works with partner agencies to find solutions so that they can commission services to improve independence. We recognise the complexity of the service provisioning and through developing partnerships and creating partner agencies we can come together to develop quality solutions.

Outcomes UK provides needs-led, tailored solutions that are both cost-effective and focused on sustainable improvement/change.

Contract set up meetings are held at the outset to ensure induction into your organisation and a shared understanding of the aim and required outcomes for any project.

Every contract is overseen thereafter by our Quality Assurance division which ensures that we are working to deliverables and timescale. This offers certainty to our clients and an extra level of challenge and support to our assigned associate which adds to our effective delivery.
