Background Information

This project has been jointly commissioned by a Borough Council, Adult Social Care, Children’s Services and Supporting People in order to investigate the feasibility of an integrated transitions service for young people with disabilities aged 13-25.

Purpose

To carry out a service evaluation in order to create an improved effective model for service delivery to young people/adults.

Aims and Objectives

To research relevant current policy, guidelines, legislation and trends in respect of the preparation for and transition into adulthood; considering research and good practice within the area of transitions and be able to understand present and future ways of responding to this group.

To understand the local response and service delivery to disabled young people in order to consider the present position, strengths and areas for development.

To propose and present an improved effective model for future service delivery including understanding future housing related support needs and aspirations for young people. The model will consider both effectiveness and efficiencies.

Activities Delivered

Outcomes UK have undertaken the following tasks in order to fulfil the service specification:

  • Held workshops to inform parents and professionals about the project
  • Consultation with parents and professionals via questionnaires, 1:1 interviews and group sessions
  • Analysis of consultation findings
  • Researched national developments in transitions
  • Ran a practitioners’ mapping workshop on pathway planning
  • Established baseline data and trends
  • Provided overview of relevant legal framework
  • Identified gaps and areas for improvement and made recommendations on the key criteria for a successful transition service.

Conclusions and Recommendations

5 option models were presented to the Borough Council on the way forward proposing an improved effective model for future service delivery taking into consideration both effectiveness and efficiencies including understanding of future housing related support needs and aspirations for young people.
